67 year old male with history of Diabetes Mellitus (Type II), CVA, and Hypertension under remote monitoring in February 2021. An alert was received for high blood pressure (systolic of 170 mmhg).
A unica care team member contacted the patient and his wife within 6 minutes of the alert. The care team member assessed with the patient’s wife how the B/P measurement was recorded, completed a hypertension medication compliance review, and inquired about any symptoms. Notes from the patient and wife encounter were entered into the UNICA system, and an alert was triggered to the attending physician.
Patient repeated blood sugar which generated alert to the care team with a reading of 245 mg/dl. Another call was placed to the patient who had taken his medication 30 mins pre meal and the test 15 mins pre meal. The patient reported no hyperglycemic symptoms. All other parameters were normal. Care team member asked about the expiration date on the hyperglycemia medications which were in date. Patient was told to call us if any symptoms occurred, and to perform a fasting glucose test in am. Patient was also advised to take the medication with his meals.
The physician followed up with the patient and changed their medications. Follow up was completed with the patient the next day emphasizing compliance with taking subsequent B/P readings to evaluate the impact of the new medication on the patient. The patient’s B/P came down to 135 mmhg systolic within 48 hours. The patient’s wife stated the following to the care team member: “ this is a very efficient service, it’s so nice to know we are been watched” (EON, FEB 2021).
