Career at UNICA | Care Coordinator

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide continuous support and education to patients regarding the use of our user-friendly Unica app and bluetooth paired devices provided to them by their Primary Care Providers (BP, Scale, Pulse Oximetry and Blood Sugar readings) based on their chronic care conditions.
  • Educate and Encourage patients to send vitals 4-5 times per week using the Unica app and maintain proper monitoring of patients as ordered by Primary Care Providers. Monitor patient vital signs using our Unica online dashboard and respond to critical/abnormal vital signs readings in accordance with company protocol and agreement with each physician practices.
  • Assess educational needs and health literacy for each individual patient.
  • Provide general health tips, exercises, and goals related to their chronic conditions.
  • Fulfill a quota of at least 200 patients.
  • Properly assess patient’s symptoms and answer all their questions and concerns appropriately via phone or text with guidance from our clinical team leader/instructor (Nurse of Medical Director).
  • Coordinate office appointments for patients to see their Primary Care Provider as needed.
  • Efficient and Proper documentation in our EMR - UNICA system/dashboard.
  • Patient health couch to live a healthier lifestyle and receive optimal results in patient monitoring programs.
  • Develop a trusting and professional relationship with each patient.

QUALIFICATIONS, KNOWLEDGE, AND SKILLS:

  • 1-3 years customer service, call center or medical office experience preferred. *MA and CNA certification required, *College Degree a plus
  • Excellent in EMR platforms (PCC, Cerner, Eclinicals, Epic, etc.).
  • Basic knowledge of chronic care conditions and disease processes.
  • Ability to troubleshoot Wi-Fi techniques via phone in order to connect our patients to our Unica app with their bluetooth devices.
  • Ability to use critical thinking skills.
  • Knowledge of medical terminology and normal vital signs range.
  • Great work ethic and a desire to provide high quality outcomes (reports) to our customers and patients.

  • Manages to achieve established standards for productivity, quality/accuracy, and patient satisfaction and overall customer service.
  • Excellent organizational and interpersonal skills.
  • Excellent verbal and written communication skills; ability to effectively communicate with physicians, patients, staff, payers, and executives.
  • Ability to investigate, analyze and resolve issues at a high level; work efficiently and accurately; and organize and a work plan.
  • Ability to work effectively with individuals of all cultures, generations, and levels of authority.
  • Flexibility to work additional hours and ability to work under stress.
